The scrotum_flap is a trend wherein the scrotum is depicted with loose extra skin, often added to the bottom middle of the sac and is meant to represent the testicles being tugged up away from the bottom of the bag, not to be confused with retracted balls which is an anatomical feature that arises when the testicles tug up independantly of the scrotum, usually at the moment of ejaculation.

This feature is commonly seen in ai misunderstanding the shape of the scrotum and being copied wholesale by artists using ai as reference.

Eventually this became a popular trend on twitter, however rather than the anatomical property it simply is additional skin added to the bottom middle of the bag with no visible purpose.
