Swelling is a vague term, but roughly refers to a bodily area actively being filled with a liquid, or other substance that would fall under inflation such that there is a visible distension. A knot will swell when blood vessels expand as blood enters the penis. As a sound effect, it may generally refer to expansion overall.

Swollen is the end state of the swelling process. A part that is swelling, should not be tagged as swollen. A knot probably should not be tagged as swollen in any situation.

Generally speaking, bloated is more common, and usually exclusively tagged for a swollen belly instead of swollen.
