It states that if a character is being topless with breasts, it should be considered questionable, but what if said character suddenly mentions genitals as well, for example by saying “you’re the one that took your dick out”?

Would it then be considered explicit, or still only questionable?

Edit: I forgot to mention that said character was fully nude in a page earlier, but on the next page only the top half is visible on the page, while the rest is now cropped out.

That is correct. Text doesn't matter, no matter how explicit the text gets.
