hand_in_clothing sounds like it could also apply to pockets or in between layers of clothing, unlike hand_under_clothing which gives the impression of being in direct contact with the body.

This isn't the case in practice, though. There aren't many tagged images, so you can clearly see that hand_in_clothing is used exclusively for hand_under_clothing scenarios. We have hand_in_pocket for pockets, which doesn't have any overlap in use.
