The wiki page for penis clearly states that the tag should only be used when the penis is directly visible, and covered yet detailed penises should instead use penis_outline.

How are you supposed to specify the size of a covered penis when the size tags (small_penis, big_penis, etc.) automatically implicate penis?

with bulge tags like huge_bulge, instead. penis_outline implies bulge, so you can use size tags that also imply bulge that don't conflict with the former's restriction.
