When I hear the term penis hat, I think penis-shaped hat rather than a hat on a penis. Although looking at the wiki I see that penis_hat is used for both.

That only makes the tag more vague, it can be easily confused with a penis shaped hat, or thanks to "boob_hat", can be confused with a penis on a head.
